Authorization for Medication Administration by School Personnel
Immunization Rates 2016-17
This form is used by parents and guardians to give school staff permission to administer medications to their students. Parents can download and print this form, fill it out, and send it to school when their child has a new or changed medication that will be given at school.
Here is a reporting of the 2016-17 immunization rates for students served in SOESD Special Education Services in Jackson County and in SOESD Early Childhood Services in Josephine County. Early Childhood Services
Early Childhood Services is a program of the Southern Oregon Education Service District. We serve families with children birth to five years old throughout Josephine County. Our inclusive toddler and preschool classrooms offer engaging learning and social experiences in a supportive atmosphere.

Child Care Resource Network (CCRN) offers services to the early learning community in Jackson, Josephine, Klamath, and Lake Counties. CCRN offers child care providers a wide range of resources, scholarships, and training opportunities to support high quality early learning programs and helps parents connect to services. CCRN is part of a nationwide early care and education network and acts as a strong voice on behalf of children and families.
Home schooling is an exemption to the compulsory school attendance law and as required in ORS 339.095, parents must notify the ESD of their intent to home-school their children. The Southern Oregon ESD superintendent’s office provides information to parents and school districts regarding home-schooling laws and regulations and registers home-school students.
