Stay Registered When You Move

WHEN MOVING TO A NEW LOCATION (adapted from Form CCD 274 1/05)
 
Be aware, once you have moved to a new location, your registered family child care license is no longer valid. You must immediately reduce the number of children in your care to no more than three child care children.
 
Call Child Care Division and request an application form to reopen at a new location.
Have the following information available when you call:
         Your Registered Family Child Care license number
         Your move date or approximate move date, if known
 
Review your training certificates and make sure your food handlers, CPR and First Aid cards are current.
         Contact Child Care Resource Network if you need to renew your cards
         You will be required to have your 8 hours of training for reopen. The 8 hours will be prorated for license periods of less than 2 years. Be sure to communicate with CCD to make sure you have the requirements met.
 
An application fee of $30 is required at the time you submit your application to reopen at a new location. NOTE: If you meet federal poverty guidelines, your fee can be reduced to $15 with the appropriate documentation. Call CCD for more specific information.
 
Once the application has been processed, a registration specialist will be calling to set up an appointment for a Health and Safety Review at your new home. Your license will become effective the day after you pass your inspection and will be valid for 2 years.
